Pregnancy-Associated Pyogenic Granuloma of the Lip with Spontaneous Postpartum Regression: A Rare Case Report

Abstract

ackground:
Pyogenic granuloma, also known as lobular capillary hemangioma, is a benign vascular lesion
commonly associated with hormonal changes during pregnancy. Although gingival involvement is most
frequent, extragingival presentations are rare and clinically significant.
Case Presentation:
We report a case of a lady in her early 40s, who conceived through in vitro fertilization and developed
gestational diabetes and hypertension. In late pregnancy, she developed a rapidly growing, painful lesion
over the lower lip associated with spontaneous bleeding for 10–15 days prior to delivery. Clinical
examination revealed a vascular, erythematous lesion suggestive of pyogenic granuloma. The patient
underwent cesarean section and developed postpartum complications, including acute coronary
syndrome and wound gaping, which were managed medically. Surgical consultation confirmed the
diagnosis, and conservative management was adopted due to her high-risk status.
Results:
The lesion showed gradual regression and completely resolved within 3–4 weeks postpartum without
any intervention. No recurrence was observed during follow-up period of 6months.
Conclusion:
This case highlights an uncommon extragingival presentation of pregnancy-associated pyogenic
granuloma with late gestational onset and spontaneous postpartum regression. Conservative
management can be considered in selected patients, particularly when the lesion is expected to regress
and surgical risks are high. Early recognition and appropriate clinical judgment are essential to avoid
unnecessary interventions.
